I hereby authorize all law enforcement agencies, past and present employers, former churches and other agencies or organizations to release information requested to Snowy Range Evangelical Free Church of Laramie, Wyoming for the sole purpose of verifying whether the applicant is or is not suitable for work with minors. This information is for church use only and will be kept confidential.
(Release of background check is valid for 120 days after signature and date below. Results of background check will only be available to the applicant, the director of children’s ministries, the pastoral staff, and only other individuals if approved by the applicant.)
